I agree that everything I have stated in the above application is true and accurate to the best of my knowledge.

I understand that FBSCR does not ship any dog under any circumstance and I will need to make arrangements to pick up my new dog if I am approved to adopt.

I understand that, if for any reason, I can no longer care for or keep a dog that I have adopted through FBSCR I must return the dog directly to a FBSCR representative, from whom I acquired the dog, and I will sign all necessary papers for the surrender of the dog.

I understand that I will be put on a list of people who are wishing to adopt a dog.  The family that is the best match between the dog, the new home and lifestyle will be given the opportunity to adopt that dog.  This by no means indicates that any applicant is an unqualified adopter, just that a better match was found for that dog.

I do not have interest in any of the following: pet store, commercial breeding kennel, buying of dogs for resale. I have not been convicted of cruelty to animals.  I am not under suspension from a purebred dog assoc.

By signing (electronic signature) and submitting this form, I hereby agree to the above terms and conditions of this Adoption Application for adopting a dog from FBSCR.

Electronic Signature (please type full name here):

Please understand that dogs placed through FBSCR have been acquired, fed, boarded, received medical care, vaccinations, been spayed/neutered, and have been tested to be free of heartworm and contagious disease. All fees associated with the care of the dogs are paid for by donations.  Donations are always appreciated – all work done by FBSCR is funded solely by public donations. The adoption fees rarely cover the fees associated with vetting and rehom
ing dogs but do help recover a small portion of the costs associated with rescuing each dog in our care.
